Your Rights as a Patient
- Receive respectful and considerate care regardless of age, gender, race, religion, or disability
- Receive complete and current information about your diagnosis, treatment, and prognosis in terms you can understand
- Participate in decisions about your care, including the right to refuse treatment
- Privacy and confidentiality of your health information
- Review your medical records and request amendments
- Know the identity and professional status of those providing your care
- Receive a clear explanation of all charges and billing
- File a complaint without fear of retaliation or discrimination
- Continuity of care and information about referrals
- Be informed of all applicable policies and procedures
Your Responsibilities as a Patient
- Provide accurate and complete health information
- Follow the treatment plan recommended by your provider
- Notify your provider if you cannot follow through with the treatment plan
- Keep scheduled appointments or notify us in advance if you cannot
- Pay your financial obligations in a timely manner
- Be respectful to all CareQuo staff and providers
How to File a Complaint
If you believe your rights have been violated, contact our Patient Relations team at help@carequo.com or call (323) 690-1564. You may also file a complaint with the appropriate state medical board or the U.S. Department of Health and Human Services.
